In an effort to cut down on the worry of fitting a visit in a specified time frame, and to better meet your needs, rates will be assigned based on a flat rate for an all-inclusive in-home visit. See below.
~I will create an account for you and keep your protected health information (PHI) secure and encrypted to a paperless server. We will be able to communicate within this server. I will have the ability to send your information i.e. intake forms, plan of care, etc. to you or your healthcare provider while keeping your PHI secure. This server is HIPAA compliant.
Who is on the team??
The name of my practice is visionary. I would LOVE to collaborate with other IBCLCs in the area. I would also like to be able to offer education and bedside hours to those pursuing their IBCLC. If you are pursuing your IBCLC and are in need of breastfeeding support hours please contact me. I will have a consent form as part of the paperwork provided to clients to sign. If you are an IBCLC that would like to be part of the team, please contact me to see if our values and mission are in alignment!!
Home Visits
In-home consultations will be a flat rate due to the many variables that they entail and will be based upon mileage.
Up to 60 miles from my home $150.
>60 miles $175
Follow-Up:
~Most concerns can be addressed in one consultation. If a follow-up visit is needed for this concern, it must apply only to the current plan of care, not new concerns. Any new concerns will be set up as a separate and new consultation.
Follow-up rate on existing concern/plan of care: $75
Future Consultations
You will need to set up a new consultation for evaluation and referral on new or separate concerns based on the above in-home consultation rate.
Email and Phone Support
This is included with current plan of care, not new concerns.
For example: if you have a consultation for low milk supply which we resolve or are resolving and at some point down the road develop nipple pain/trauma then this will need to be set up as a separate plan of care and consultation.

Diagnose and Treat
My training, however, allows me to perform a comprehensive assessment to be able to isolate the cause of the concern or problem you are having. I will also be able to provide you with discharge paperwork and a written plan of care. If the concern needs a medical intervention, I will be able to refer you to your physician for medical diagnosis and treatment based on my assessment.
Make decisions for you
It is my hope that you understand that MY goal is to help you reach YOUR goal. I will provide you with evidenced-based education so that you can make an informed decision. Meaning you will know all sides of how your decision will affect your goal so that you remain in control and know it is always your choice.
Speak to your Physician on your behalf
I will practice within my scope and if needed can send the contents of your chart i.e. health history, consultation assessment, plan of care, etc. to your Physician of choice.
Project any of my own beliefs onto your experience
Breastfeeding is a personal decision. There are many routes to take to provide breastmilk to your child. My thoughts, opinions, and personal breastfeeding experience may vary from yours. My job is to help you reach your goal by giving you the tools, assistance, and information you need to help your breastfeeding journey continue so you can reach your goal.

Payment Options
If you have Blue Cross Blue Shield or Cigna then use the link below. This will confirm your coverage based off your policy. Upon confirmation of coverage, you will be contacted to proceed with setting up a consultation. This will allow for your insurance company to be billed directly, alleviating you of the stress of the up front fee for service and seeking reimbursement. Here is the link:

For all other clients that do not have the above insurance, then I am only able to take Cash, Debit/Credit, and Paypal only. Payments must be received at or before the time of service. At this time I am unable to bill insurance directly for you. I will provide you with a Superbill with proper insurance codes for you to submit to your insurance company for reimbursement. Reimbursement depends on the insurance company and plan that you have in place. Contact your insurance company to see what your plan covers. Ask about breastfeeding support/counseling/consultations and benefits, including supplies for breastfeeding i.e. pumps, nipple shield, pump parts, etc. Many insurance companies will reimburse part or all of the cost of a lactation consultation and supplies, especially one based on a physician referral. There may be a form your insurance provider requires for you to fill out to be faxed with the Superbill that I provide you.
